DescriptionWater colour drawing of a section of the Jejunum showing a diverticulum. From a female aged 61 years, with a history of peritonitis 40 years ago. Vague symptoms of dyspepsia. During the last two weeks she had attacks of acute lowe abdominal pain with occasional vomiting and a more severe attack one day previous to admission. On examination there was tenderness and rigidity in the lower right quadrant and towards the middle line, with some central abdominal distention. At loporotomy a jejunal diverticulum was found about 6-8" below the duodenal-jejunal junction. It was adherant to the mesocolon and around it was a localised peritonitits. An incision was made over the swelling in the mesentery as the nature of the lesion was not certain and a large concretion was removed. The affected portion of jejunum was resected and the remaining ends of small bowel united by a lateral anastomosis. The segment of jejunum measuring 16cm in length is considerably dilated. Extendeing into the mesentry there is a hemispherical diverticulum with a wide base measuring 4cm. The wall of the diverticulum is thick, oedematous and inflamed, anfd the peritoneal surface over it is covered with inflammatory exudate and recent blood clot. The faecal concretion removed from the diverticulum is brownish in colour and appears to be soft and pliable.
