A female from Togoland, West Africa, developed in the left loin a small subcutaneous swelling which slowly enlarged. The swelling was painless, soft and fluctuating and might have been mistaken for a lipoma. or a cold abscess arising from the spine. She gave a history of guinea worm in the right leg two years previously. Under local anaesthesia the swelling was incised and odourless pus escaped. The granular pigmented lining characteristic of guinea-worm abscess revealed the nature of the lesion. The abscess was excised and as usual healing occurred per primam.
The abscess which is considerably shrunken due to fixation has been laid open to show the pigmented, granular inner surface. The wall of the abscess is only 2 or 3 mm in thickness. A fragment of calcified Dracunculus parasite, removed from the groin of another patient, has been .placed in the abscess cavity.
