The loop of jejunum which measures a little more than a metre in length has been filled with fluid to restore the appearance of the bowel seen at operation. The bowel is distended and congested but the. peritoneum remains smooth and glistening. Numerous diverticula protrude from the attached border of the bowel, between the two peritoneal coats of the mesentery. They vary in size from less than 1 cm. to 5 cm. in diameter. The site of origin of the diverticula is at the point of entry of the larger mesenteric vessels.
From a male aged 62 years. For ten years there had been transient attacks of colicky pain in the umbilical region. Four days before admission the colicky pain came on acutely and persisted. He vomited once. The bowels remained regular. Twenty years previously he was operated on for left inguinal hernia. On examination there was marked central abdominal distension and there was tenderness in the left hypochondrium but no guarding. Loud borborygmi were heard but no succussion noises could be elicited. A radiograph with the patient propped up showed several fluid levels, apparently in the dilated small bowel. At operation a congested loop was kinked over an omental band attached to the parietal peritoneum at the site of the old herniotomy. The band was divided and the loop of inspection was found to possess numerous diverticula along its mesenteric border. No diverticula could be seen in the bowel above or below the loop. Though the bowel appeared to be viable, the loop was resected and, continuity established by a side-to-side anastomosis.
