The contents of the middle ear have been removed, the mastoid air-sinus completely gouged out, and through the tympanic antrum the posterior fossa of the skull has been opened through an oval area 20 mm. in length by 12 mm. in breadth in the neighbourhood of the sigmoid sinus. The transverse sinus is blocked by a large firm blood clot. Through a small opening in the teen tympani the middle cranial fossa has been entered, and a corresponding area of the dura mater removed. Around this opening there is epidural blood on the dura mater and on the bone.
