DescriptionGanglio neuroblastoma of mediastinum. From a male aged 18 years in whom a mediastinal tumour was discovered in a Mass Radiography survey.
The specimen consists of a roughly ovoid nodule measuring 9 x 5.5 x 4.5 cm. About half of it is covered by a serous membrane. It is composed of soft, grey-white tissue with fibrous trabeculae and occasional areas tippled by yellow necrotic material. One subcapsular zone 3 cm. in diameter is completely necrotic and haemorrhage has occurred in this region.
The structure of this tumour is not well preserved and although different portions of tissue have been sectioned on several occasions it has been impossible to obtain entirely satisfactory histological preparations.
The tumour is a ganglio neuroblastoma composed of neural tissue supported by fibrous trabeculae and amylinate nerve fibres. There are several small foci of calcification in the stroma. The intervening tissue which tends to disintegrate is largely composed of rather sparsely distributed cells in a loose acidophilic matrix resembling brain substance. The cells include numerous late neuroblasts and immature ganglion cells - some of them multinucleated - and occasional rosettes can also be seen. Occasional small areas are much less mature and are composed of closely-packed lymphocyte-like neuroblasts. The tumour must therefore be regarded as malignant and the possibility of metastasis cannot be excluded, despite the evidence of differentiation elsewhere.
