DescriptionIntrathoracic ganglio-neuroma.
A male aged 6¾ years was investigated as a tuberculous contact and a radiogram showed a shadow in the upper part of the chest. The opacity was rounded and lay in the posterior part of the mediastinum behind the trachea and above the left main bronchus. Through a left thoracic approach the tumour was found on the upper left ribs and projecting into the thoracic cavity. The tumour was removed with a portion of parietal pleura. A nerve was found running into the tumour.
The tumour is piriform, measuring 5 by 4 cms. The surface covered with parietal pleura is flat, the remaining surface is bossed and where not obscured by fatty connective tissue shows a well-defined capsule. The cut surface of the tumour is pinkish and semitranslucent.
Histological investigation showed a thick capsular zone consisting of loose fibrous tissue in which lie scattered large cells with wide granular cytoplasm containing a rounded nucleus. Inside this denser capsular zone there is loose oedematous tissue with similar cells and also clumps of smaller cells. There is a great deal of degeneration obscuring the typical cell pattern. The smaller, immature cells are not very irregular and there are few mitotic figures. The ganglio-neuroma is of the well differentiated cell type and presumably therefore relatively benign.
