From a female aged 18 years. She was investigated because she lacked energy and was losing weight. The pre-operative diagnosis were malignant teratoma, or malignant lymphoma, or possibly an infected dermoid.
Left thoracotomy, which is where an incision in the chest wall is made, was performed. The mediastinal tumour invaded the left upper lobe which was resected with the tumour. The tumour ulcerated on the inner surface of the pericardium and most of the left side of the pericardium was resected. The right pleural space was widely opened. The left innominate vein was invaded and was resected with the tumour.
The specimen consists of the upper lobe of the left lung at the apex of which is situated a lobulated tumour. The relationship of this tumour to the lung is less evident than at operation. The lower part of the lobe is congested and less well aerated than normal. At the extreme apex a line of cleavage exists between the tumour and the lung which here possess only an anatomical relationship. At a lower level the tumour and the lung are fused and on the cut surface where the tumour has a lobulated outline although fairly well defined the appearances would be consistent with a tumour arising within the lung substance.
