DescriptionThymus showing myasthenia gravis from a female aged 5 years, in whom bilateral ptosis and hypotonicity had been noticed since birth.
The child was seen at the age of 3 and had not learned to walk and standing up had been delayed. A therapeutic trial of prostigmin resulted in dramatic improvement, confirming the diagnosis of myasthenia gravis. Thereafter prostigmin was given regularly with fair success. As the Education Authority was reluctant to accept her at an ordinary school, the question of thymectomy was raised. Though the results in young subjects had previously not been encouraging it was decided to operate and they found the thymus was enlarged; the gland weighed 36 grams which is slightly larger than normal for a child of 5.
Immediately after the operation her expression and eyes seemed much more normal than before. Thereafter the child remained active and well on a dose of prostigmin slightly smaller then before operation and two years later the improvement after operation was still maintained.
