DescriptionAortic arch laid open, with heart, greater portion of right lung and portions of the oesophagus and descending aorta, to show an aneurysm.
The right and left ventricles have been laid open and are normal. The aortic valves appear healthy but at the commencement of the aorta there are one or two small atheromatous plaques, especially in the neighbourhood of the right posterior cusp. The aorta is distended 2.5 cm from the aortic valve into a fusiform cavity that measures 8 x 5 cm in diameter. The aneurysmal wall is somewhat rough and irregular and is coated with fibrinous deposit. The lung surface shows a good deal of chronic and recent pleurisy and in the substance of the lung a small irregular cavity occupies the apex and in the inferior lobe there are numerous irregular spaces. Microscopically the cavity at the apex of the lung is tuberculous, the other irregular cavities are bronchiectacic.
Posteriorly the sac of the aneurysm has been opened and demonstrates laminated, and organising clot within.
