From a male aged 23 years, who was given a preliminary diagnosis of a ventricular septal defect at age 5. Continued to experience bouts of breathlessness in the following years. At age 19 he was re-examined and a E.C.G. showed well marked left ventricular hypertrophy. Radiographs confirmed the presence of marked cardiac enlargement which was generalised with gross enlargement of the main pulmonary artery. The provisional diagnosis of aorto-pulmonary artery septal defect was considered more likely.
At 23 years of age he was admitted to hospital orthopnoeic and in gross congestive heart failure. Cardiac pulsation was so vigorous that it shook his whole body and the bed clothes covering him. There was a continuous murmur over the base of the heart and up to the left infraclavicular region. Cardiac catheterisation showed the presence of a ventricular septal defect with pulmonary hypertension. The patient died aged 23 .5 years of congestive heart failure.
On post mortem it was confirmed there was a septal defect, functional tricuspid incompetence and aortic incompetence, this later, however, being due chiefly to a perforated aneurysm of the adjacent sinus of Valsalva which had erupted through the wall of the infundibulum as well as herniated through the ventricular septal defect into the right ventricle. Also, the lungs were grossly congested.
This specimen shows a case of perforated aneurysm of the anterior sinus of Valsalva which has herniated through the adjacent myocardium into the right ventricle and a large ventricular septal defect, presenting clinically with a continuous murmur and thrill and leading to death in congestive heart failure.
