DescriptionAneurysm of the heart, from a male aged 78 years.
The patient was admitted to hospital on account of an oesophageal carcinoma, and submitted to have this tumour removed by operation. He died four days later having developed bilateral pleural effusion and excess fluid in the lungs.
At post mortem the lungs were found to be markedly congested and oedematous. There was some pleural effusion with slight collapse of the lower lobes. The line of the operative anastomosis appeared satisfactory.
The specimen consists of the heart opened to expose the interior of the left ventricle and aortic valve. The heart is abnormal in shape due to an aneurysm of the left ventricle. There is considerable enlargement of the heart due to dilatation and hypertrophy of ail chambers. At the apex of the left ventricle is an aneurysm measuring 5 - 6 cm in diameter with a fibrous and partially calcified wall. It is largely filled with old laminated thrombus.
To the outer wall of the aneurysm is a portion of densely adherent parietal pericardium. (At post mortem, the pericardial sac was found to be obliterated by adhesions, most dense however at the apex). The pericardium over the remainder of the heart shows numerous adhesions and a considerable amount of subpericardial fat. The subpericardial vessels are prominent and unduly tortuous.
The myocardium of the left ventricle shows well marked hypertrophy adjacent to the mitral and aortic valves but becomes thinner as it is traced towards the apex and ends at the orifice of the aneurysm. The muscle coat is
blotchy and discoloured by numerous areas of congestion or minute haemorrhages. In some areas there has been fibrotic fatty infiltration of the muscle in the subpericardial zone.
The endocardium is thickened in the apical segment and this is demonstrated by the opaque white appearance. The coronary arteries are markedly atheromatous. There is slight calcification of the aortic cusps but no stenosis or incompetence. The left atrium is grossly dilated and the mitral valve dilated to a degree indicative of incompetency. The right atrium is also dilated.
