From a female aged 70 who for one or two years had loss of memory and who recently had become mentally confused. She was admitted with the provisional diagnosis of bronchopneumonia and left sided cardiac failure. She was obviously very ill being dyspnoeic and cyanosed. The pulse was very feeble and Irregular and death occurred before any special investigations could be carried out.
At post mortem there was acute myocardial infarction with massive mural thrombosis and a large haemopericardium due to rupture of the myocardial infarct. There was also an old fibrosing myocardial infarct which was giving rise to cardiac aneurysm. The interventricular branch of the left coronary artery was extremely atheromatous and bad apparently been previously thrombosed and recanalized. The minute central vessel was now occluded by recent thrombus. The lungs showed congestion and oedema and the bronchi contained muco-pus, but there was no obvious pneumonia. The pleural sacs each contained 600 cc. of fluid. The kidneys were slightly reduced in size and showed marked arteriosclerosis but no evidence of hypertensive disease. The abdominal aorta we markedly atheromatous. A subdural haematoma was present on the left side covering the posterior half of the frontal lobe and the whole of the parietal and the upper part of the temporal lobes. There was no fluid blood remaining, but between the dura and the arachnoid there was a thick fibrous walled space filled with tissue stained brown by haemosiderin. There was general cortical atrophy and the lateral ventricles were moderately dilated. No infarcts or areas of softening of the brain were seen. The basal arteries were atheromatous and narrowed.
