From a male aged 24 years in whom extraction for dental caries eighteen months previously had included all maxillary except the left premolar and, molar teeth. Two months ago swelling of the left maxilla was noticed and. a week later was discovered a sinus opening at the facial aspect of the alveolar process and discharging foetid pus. The maxillary air-sinus was opaque to transillumination and radiography showed an osteoma. The odontome occupied a purulent cavity and the maxillary air-sinus was filled with gelatinous polypi which were removed and intranasal drainage established.
The odontome, 25 mm. in maximum diameter, forms an irregular, somewhat quadrilateral amalgamation of bone and dentine with many rounded spicules, nodular and superficial masses of enamel some of which suggest the cusp-formation of the crowns of teeth.
