From a female aged 47 years with no history of rheumatic fever or growing pains. There had been recurrent bronchitis for twenty-five years, breathlessness on exertion for fifteen years, recurrent small haemoptysis for six years, and recurrent episodes of congestive heart failure for five years. Examination at the age of 45 revealed considerable cardiac enlargement, auricular fibrillation, B.P. 150/74, a basal systolic murmur conducted into the neck, an apical systolic murmur and a long blowing diastolic murmur down his left sternal margin. There was no electrocardiographic evidence of preponderant hypertrophy of either ventricle. On radioscopy there was prominence of both aorta and pulmonary artery with enlargement of left atrium. She was not considered suitable for mitral valvulotomy. Examination at 46 years of age revealed a harsh mitral systolic murmur which was louder than a faint mid-diastolic murmur also present at the apex. There was a loud para sternal diastolic murmur, The apical impulse lacked the abrupt quality of right ventricular hypertrophy, and the first mitral sound was not loud, or snapping. The mitral lesion was considered the dominant valvular lesion, but she was rejected for valvulotomy on the evidence of gross cardiac enlargement with recurrent congestive failure without the clinical features of tight mitral stenosis.
At 47½ years of age she died from bronchopneumonia and congestive heart failure. Examination during the last few days of life revealed the thrusting apical impulse of left ventricular hypertrophy, a variable diastolic thrill at the apex and a widespread systolic murmur. The pulmonary second sound was not accentuated. Mitral valve disease was again considered the dominant lesion
