A mentally and physically distressed patient is seated on a high stool. He cluthes his right leg whilst pressing his right foot against the operator, who is holding the victim's jaw and using a key to extract a mandibular left tooth.
The engraving looks very similar to an engraving by James Gillray (1757-1815). Many of Gillray's caricatures were issued under fictitious names so this could be one of his.
