The preparation is the distal portion of the tibia and fibula with the talus
From a male aged25 years in whom an alleged severe contusion nine years previously by a falling mass of coal incapacitated him for work during two months and was succeeded by pain. The joint was immobilized in plaster of paris during nine months with some improvement. Three years later massage enabled him to take light work which did not entail walking. Four years ago swelling and pain necessitated a period of rest and recently it has been acutely painful in walking and radiography showed disorganization of the joint. Married, two healthy children, Wassermann blood reaction negative. Dissection after amputation showed much amorphous cretaceous material which penetrated along the tendon sheaths and into the cellular tissue about the joint. Microscopic examination showed giant-cell and epithelioid-cell formation with caseation necrosis, perivascular inflammatory cell reaction and some endarteritis.
By its lateral articular surface the talofibular articulation is replaced by bony union. The talotibial articular surfaces have been wholly destroyed and the subjacent bone is irregular and porous. Decalcification and porosity of the distal end of the tibia has been limited by the attachment of the articular capsule and the interosseous ligament of the tibiofibular syndesmosis. Little remains of the surfaces for articulation with the calcaneum and the destruction of the cartilage of encrustation of the head of the talus exposes its cancellous bone.
