From a female aged 51 who had noticed a tumour near the left deltoid insertion twenty-three years previously. Nine months after this the tumour, an exostosis, was chiselled off. Sixteen years later local recrudescence was noted. The tumour which was attached to the bone but not to the scar, grew slowly during four years and rapidly during one year. Biopsy showed it to be a giant-cell tumour and deep radiotherapy was applied. Three months later the limb was disarticulated at the shoulder. Eight months later, i.e. about four months before death, the presence of pulmonary metastasis was noted.
The left lung contains a somewhat kidney-shaped metastasis, obliquely placed and approximately 17 cm. in length. A small node lies close under the pleura. The large metastasis is composed of firm, greyish, solid tissue and contains a number of spaces or cysts, some filled with blood clot. It has a well defined margin and in places is surrounded by a false capsule. The microscopic appearance of the tumour removed by amputation was, in places, that of a simple giant-cell tumour of foreign-body giant-cell type, with the usual fibroblastic stroma; another part contained more foreign-body giant-cells and osteoid tissue but the amount of mitosis justified the name “sarcoma”; a third part showed, in addition, monster hyperchromatic cells with numerous and irregular mitoses, and round and spindle-shaped cells of malignant character. The pulmonary metastasis showed a giant-cell tumour of fibroblastic type, exactly as in simple giant-cell tumours. The giant-cells had a peculiar perivascular distribution. Osteoid tissue was present, and in addition there was true bone in places. The adjacent pulmonary alveoli were filled with desquamated alveolar epithelial cells. In a bronchus the epithelium appeared to be forming giant-cells.
