Posterior to the femur the popliteal artery is distinguishable with the popliteal vein laterally and superficially. The central of the three muscles posteriorly is the semimembranosus, and superficial to it lies the tendon of the semitendinosus. Medial to the semimembranosus muscle is the sartorius and between them at a more superficial level, the gracilis tendon. Lateral to the semimembranosus muscle is the biceps muscle, and between them at a deeper level is the tibial nerve with lateral to it the common peronaeal nerve. Anterior and lateral to the femur a yellowish fleshy neoplasm lies deep to the vastus medialis, the quadriceps tendon, and the vastus lateralis, all of which are raised by the tumour mass. The lateral segment of the tumour is homogeneous, the medial segment is rough and granular and contains an oblique haemorrhage, these features having apparently followed a recent biopsy the incision for which marks the skin anteromedially. The anterior surface of the femur has been decalcified and destroyed by the tumour which replaces the bone marrow. Medially the femur has been perforated and the tumour passes subperiosteally to the popliteal surface where its vascularity has superficially decalcified the compact bone. The tumour is a spindle-cell sarcoma and of periosteal origin. The distal cartilage of conjugation is still intact.
