DescriptionClavicle showing a fibrolipoma, associated with a congenital bifidity. A fibrolipoma is a type of benign tumour that arises from fatty tissue, and here it is centered on a type of variant anatomy where the clavicle is bifid, or slightly 'forked', towards the distal (shoulder) end.
The preparation is a right clavicle from a male aged 12 years, of Southern China (Swatow), in whom the swelling was alleged to have been present during only two years. The tumour projected from the anterior surface of the clavicle towards its lateral extremity, was soft on palpation and fixed to the bone. The skin over it was freely movable. It seemed to have a central hard core. The clavicle was excised subperiosteally under the impression that the tumour might be sarcomatous. Convalescence was uneventful. No deformity or interference with movement followed the operation. A radiogram of the tumour shows a subsidiary lateral portion of the clavicle arising about midshaft and directed cephalically into the tumour. (N. D. Fraser. China Med. Journ. A well-defined, lobulated, fatty-looking mass enclosed in a delicate capsule projects from the superior and anterior surfaces of the lateral half of the clavicle. It measures 180 mm. in circumference and on section shows a homogeneous yellow fatty-looking surface. About one-fourth of the tumour has been removed by dissection to show its osseocartilaginous core. Microscopically the tumour shows the structure of a fibrolipoma and there is no evidence of malignancy. A decalcified portion from the centre shows an osseocartilaginous structure,The cartilage is undergoing calcification and replacement by bone. Some haemorrhage near this portion of the tumour may have been produced by manipulation during the removal of the clavicle.
