Amputated from an individual in whom acute septic osteomyelitis of the tibia had commenced nine years previously. The septic affection of the bone had been followed by extensive ulceration of the leg.
None of the epiphyses has joined with the diaphyses. Most of the distal half of the diaphysis of the tibia is absent following necrosis. The proximal portion of the diaphysis distal to the tibial tuberosity is irregular and increased in size. Anteriorly the surface forms an irregular shallow excavation increasing in depth and porosity towards its distal extremity. Posteriorly the surface shows slight osteophytic overgrowth. The distal portion of the diaphysis has been greatly decalcified and absorbed to within some 20 mm. of the distal cartilage of conjugation. Posteriorly the surface is osteoporosic.Laterally a few osteophytes of light porous bone pass anteriorly towards the fibula. The proximal 80 mm and the distal 30 mm. of the diaphysis of the fibula are normal in texture. The rest of the diaphysis has undergone extensive decalcification from the adjacent hyperaemia and is osteoporosic with a considerable new formation of irregular light porous bone which is more in evidence posteriorly and medially than anteriorly and laterally at the site of the ulceration of the soft parts.
