TitleRight tibia showing changes due to chronic osteomyelitis
DescriptionTibia, an adult right, with the fibula, showing changes due to chronic osteomyelitis.
From an individual who suffered during twelve years from ulceration of the legs which was bilateral and was treated as a manifestation of syphilis.
In the proximal two-thirds such a chronic periostitis as might be caused by septic infection from an ulceration of the soft parts, has produced rarefaction of all the surfaces. On the tibial crest, the medial and to a less extent on the lateral surface a subsequent osteomyelitis has resulted in the formation of a large, irregular and in places porous, sequestrum. The distal third of the diaphysis is increased in circumference and its compact bone more or less rarefied. Towards the distal extremity the diaphysis on its posterior and lateral surfaces is irregularly destroyed by decalcification. The fibula remains unaffected. An oval cloaca in the popliteal surface of the tibia discloses an underlying sequestrum of cancellous bone. The appearances of the sequestrum have been modified by long treatment and by the application during years of preparations probably mostly mercurial.
