The preparation is the left tibia with the fibula and a portion of the femur showing acute septic osteomyelitis.
From a child in whom amputation was performed eight months after the onset of the illness.
The septic affection has commenced at the proximal tibial metaphysis where subsequent separation between the epiphysis and the diaphysis has occurred. The cancellous bone of the epiphysis has been destroyed and the epiphysis with the joint flexed at a right angle has perforated posterior to the knee joint. The inflammation has produced some decalcification of the femur outside the joint capsule. Two thirds of the tibial diaphysis forms a smooth sequestrum undergoing decalcification and absorption at its proximal extremity. The distal third of the tibia is surrounded by fibro-osseous involucrum with a single crateriform cloaca towards the medial malleolus. The involucrum is prolonged posteriorly at some distance from the necrotic diaphysis. The fibula is unaffected except that it has been softened and shows an impacted greenstick fracture at its neck.
