The fracture has passed through the distal third of the diaphysis obliquely distally and laterally and through the fibula transversely 45 mm. proximal to the apex of the lateral malleolus. The distal fragments have been tilted forwards distally so that the proximal end of the distal fragment of the tibia makes an angle salient posteriorly and the proximal end of the distal fragment of the fibula projects posteriorly clear of the proximal fragment to which it is united by a narrow irregular bridge of bone. Anteriorly close to the epiphysis a triangular defect exposes the marrow cavity of the tibia and the rounded margins of this opening suggest removal of a comminuted fragment by operation. Arthritis of the ankle has accompanied the healing and the inferior articular surface of the tibia is joined by osseous ankylosis with the trochlea of the talus, and the articular surface of the lateral malleolus is ankylosed anteriorly to the lateral articular surface of the talus.
Old compound fracture lower end of the tibia and fibula. Oblique fracture of the lower tibial fragment is recent with involvement of the ankle joint with partial subluxation of the talus.
