DescriptionDistal portion of an adult left thigh showing a compound fracture of the femur, from a soldier who died in France 1916. There is no evidence that the fracture has been caused by gunshot.
The fracture has passed obliquely distally and laterally through the diaphysis just proximal to the popliteal surface. There has been some overlapping, the proximal fragment being displaced laterally. To retain the fragments in apposition they have been bound together by two circles of wire. No union seems to have occurred though the breadth of the marginal cicatrix indicates survival during several weeks. About 80 mm. of the femur lies exposed in the floor of aneliptical ulcer and for the most part is covered by granulation tissue. Marginally the cicatrix is adherent to the bone. On the proximal part of the proximal fragment a thin scale of striated and partially decalcified bone is the remains of a superficial sequestrum of the compact bone. A bare necrosed area can also be seen towards the proximal end of the distal fragment. Medially and posteriorly are cicatrices of healed vertical incisions.
