From a professional mendicant Hindu aged 45, the affection of whose left foot had persisted daring ten years. The disease commenced as a white patch on the sole of the foot which he ascribed to the use of stilts in walking. The patch later became fissured and exuded a glutinous discharge in which small oval uniform bodies were of ten observed. The swelling of the foot gradually became generalized and other ulcers formed and discharged characteristic granules. A good deal of pain, relieved by the evacuation of the contents of the tumours, had attended the extension during the past year.
The foot and toes are much swollen, and towards and on the plantar aspect are numerous circular sinuses of uniform size and appearance. On section a large cavity replaces the structures of the foot anterior to the talus and calcaneum and contains some remains of decalcified bone. There is considerable subcutaneous fibrosis. The fungus which is found in the white variety of mycetoma is descomyces madurae.
