From a nullipara aged 58 years in whom sudden pain in the left foot was followed by its becoming cold and white. Fourteen days later the hallux became black, and this discoloration gradually spread. As the hallux became loose she pulled it off and later a portion of the heel fell off, and later still the foot separated at the ankle and was similarly treated, and thereafter for the first time she sought medical advice. Discoloration had spread to the middle of the leg, and eventually the distal part of the tibia and fibula separated. Amputation was performed through the distal third of the thigh, and examination showed a thrombus in the popliteal artery.
The stump is conical, the apex being formed by the tibia and fibula covered by oedematous granulation tissue. The distal 50 mm. to 90 mm. of the stump is raw and fleshy and covered with granulation tissue, and is delimited proximally by an abrupt irregular cutaneous margin. For some 40 mm. the cutaneous surface is smooth and pale, and proximally the thigh is swollen, oedematous, and the skin dirty and pigmented.
