From a female aged 17 years who died on the eighth day of strangulation of the stomach and transverse colon within the sac of the diaphragmatic hernia. The pain and vomiting were supposed to indicate gastric disturbance and the condition was only recognized post mortem. Much of the dilated stomach was contained within the sac and behind the stomach a congested loop of transverse colon showed constriction corresponding to the hernial opening. The oesophagus was displaced upwards by the hernia and appeared short but automatically returned to its proper length and position when the hernial contents were removed.
A portion of the liver has been left under the right cupola of the diaphragm. The stomach which is congested is partly exposed by removal of an oblong portion of the left half of the diaphragm. The opening of the hernial sac is within the oesophageal hiatus immediately to the left of the oesophagus. The sac is subglobular in shape, tendinous in structure and gives off by a small opening a diverticulum which passes posterior to the oesophagus and forms a spherical protuberance into the right side of the thorax close to the spine a few millimetres above the right half of the diaphragm. A small portion of the main hernial sac has been removed to show the interior and its serous lining.
