From a 20 year old private 4th Gordon Highlanders who was wounded in France 20th April 1915 by a German rifle bullet at unknown range. On arrival at the base hospital fifteen days after being wounded there , was a wound of entrance medial to the tibial tuberosity and of exit at the lateral aspect of the calf some 75 mm. distal to the head of the fibula. The leg was swollen and tense and the foot blue and cold and no arterial, pulsation could be felt. The limb was amputated proximal to the femoral condyles and subsequent examination showed the popliteal artery to have been completely torn across.
The bullet has entered the middle of the medial surface of the tibial diaphysis 50 mm. distal to the articular surface. The opening is almost circular and from its lower margin a fissure passes distally along the diaphysis for some 50 mm. With a lateral and posterior direction the wound of exit from the tibia on the posterior surface proximal to the popliteal line is large and a quadrilateral piece of bone has been carried away. From the medial border of the hiatus a simple vertical fissure extends proximally towards the medial condyle and a V-shaped fissure extends distally down the posterior surface across the medial border and up the medial surface to near the wound of entrance. From the posterior aspect of the fibula just distal to the neck the bullet has removed a notch of bone 25 mm. in vertical extent.
