Clinical history
From a soldier. The skull showed two bony defects, one in the left temporal region which marked the site both of entry of the shell fragment and the operative approach to remove same, and the other a left parietal decompression.
Description:
The dura was firmly adherent to the bone and scalp at the site of the decompression and to the underlying brain.
The base of brain and meninges were adherent and in the specimen the meninges around the optic clausma and the pedicles is covered by adhesions by which the brain stem was bound to the margins of the tentorium cerebelli. These adhesions extend round the brain stem in the neighbourhood of the foramina and Luschka and Magendi — The appearances are those of adhesions following basal meningitis.
The left parietal and temporal lobes show flattening of the convolutions and the left frontal lobe is the site of gross damage. The under surface has been torn during separation from the skull opening into cystic space from which a quantity of cerebro—spinal fluid escaped, indicating probably communication with the anterior horn of the ventricle. The cerebral tissue is extensively bruised and there has been extravasation of blood.
