From a female aged 16 years in whom recurrent discharge from a minute congenital opening had been noticed since ten days after birth, medial to the sternomastoid muscle just above the right sternoclavicular joint. The sinus (5.(3).a.1.), which passed to the direction of the tonsil, was easily isolated and excised along with an elliptical portion of skin bearing the opening.
The fistula is 106 mm. in length and tapers above to a point. The distal third is dilated and fusiform and has a maximum diameter of 15 mm. Microscopic examination shows the wall lined by atrophic transitional epithelium, and the immediate subepithelial tissue densely infiltrated with chronic inflammatory cells. The outer wall is composed of dense fibrous tissue showing slight inflammatory reaction.
