From a male aged 60 years who had experienced at birth the escape of urine from a fistulous opening at the umbilicus. When aged 8 years there was a passing attack of retention, and all the urine was passed through the fistula. Apart from this, the leak at the umbilicus was only an ooze, but recently he had attacks of burning sensation when passing urine and these tended to grow more marked, and he has noticed traces of blood in the urine from time to time. There were no other obvious signs of abnormalities. However, at operation a vertical fusiform incision incorporating the umbilicus was made down to the pubis. Without opening the peritoneum the fistula, including apex of bladder, was excised and the fundus of the bladder closed. Recovery was uneventful.
The fistula from the skin surface to the apex of the bladder is 7 cm. in length. It is surrounded by a thick layer of fat over which is a well defined fascial covering continuous with that of the bladder. The narrowest part of the fistula is where it opens at the umbilicus and this would barely admit the coloured glass rod. A portion has been removed on the posterior aspect near the middle of the fistula and also part of the posterior wall of the resected apex of bladder. The fistula is widest near its middle. The wall of the fistula taken from its mid point was found to be lined by transitional epithelium.
