Vinylite resin cast of the pelvis and renal artery of a right kidney removed at operation by Professor M.F.A. Woodruff from a man aged 57 with hypertension. This patient had a 15 year history of intermittent claudication. In 1953 when investigated, aortic thrombosis was demonstrated with the aortic block just below the renal arteries. Conservative treatment with anticoagulants was given. The patient developed an extensive collateral circulation via the mesenteric and iliac arteries to such an extent that the femoral arteries became palpable.
In October, 1959 severe symptoms of hypertension made the patient seek relief. The blood pressure reading was 300/145. An intravenous pyelogram showed a very small poorly functioning right kidney. Right nephrectomy relieved the patient’s symptoms. Five weeks after operation the blood pressure was 180/90. It has, however, risen again subsequently.
Note the small stunted pelvis and calyces, and very poor filling of the renal artery. Histological examination showed hyalinised glomeruli, atrophied tubules and fibrosis of vessels.
