From a parous female aged 57 years who thirteen years previously had an attack of urinary infection and phlebitis of the right leg. The attack of urinary infection recurred on three occasions. Six weeks ago the patient again began to have frequency of micturition and dysuria and on this occasion there was aching pain in the right loin. For some time she has been subject to headaches and has been easily tired and lacked appetite.
On examination she was pale and rather slow in cerebration. The tongue was moist and slightly furred. The temperature was normal. Blood pressure was 160/100 mm. of Hg. The fund were normal. The kidneys were not palpably enlarged. Slight tenderness was manifest over the lower end. of the right ureter. The urine was turbid and contained pus and bacilli coli. A straight radiogram of the abdomen showed an oblong shadow in the line of the right ureter at its lower end and several shadows of mesenteric calcified lymph nodes. Excretory pyelography showed a normal shadow on the left side. On the right, the outline of the kidney was pear-shaped, excretion occurred only from the larger and lower portion, and. the ureter was slightly dilated. Medial to the lower end of the ureter there was the oblong shadow seen in the straight radiogram. Cystoscopy revealed. congestion of, and a few petechial haemorrhages in, the mucosa and oedema around the urethral orifice. The left ureteral orifice was normal and a catheter was passed with ease. On the right side there was a well marked ureterocele and it was with difficulty that a catheter could be passed and only for a distance of 12 cm. The retrograde pyelogram confirmed. the findings of excretory pyelography.
