From a male aged 68 who, after twenty years’ employment as a paraffin refiner, had to give up that work on account of increasing dermatitis of both forearms, hands and feet. Four years later radiotherapy was commenced and continued at intervals during eight years. Three tumour-like growths on the left forearm and the dorsum of the right hand and foot showed, on biopsy, epithelial invasion of the subcutaneous tissue. This operation wound on the left forearm quickly formed a malignant ulcer, and the limb was amputated proximal to the elbow. Recurrence in the stump occurred four months later.
The skin of the forearm and dorsum of the band is rough from hyperkeratosis and here and there heaped up epithelium has formed little warty excrescences. Throughout the roughened skin are many areas where the surface is smooth, white and has a cicatricial appearance. On the dorsomedial aspect of the forearm just distal to the elbow is a somewhat quadrilateral ulcer, 90 mm. in its sagittal diameter. The margin of the ulcer is extremely irregular but not undermined, and distally shows a little heaping up of epithelium. On the floor of the ulcer the deeper layers of the skin have been irregularly destroyed, but there is no appearance of regeneration.
