DescriptionSquamous epithelioma arising in sinus on heel.
From a male aged 60 years, a shipmaster who had spent most of his life in the east. Thirty seven years ago in the Persian Gulf, he was bitten by a shark and the dorsum of the left foot and around the ankle was severely lacerated.
The wounds became infected and discharged for several months. During the next fourteen years the wounds remained healed and there was little trouble except for loss of movement in toes and stiffness of ankle. A hornified area on the left heel then broke down and discharged. A small patch was excised but it was several months before the ulcer healed over with hornification. There was a slight discharge ten years later which persisted for a few weeks. Two months prior to admission to hospital, the heel began to discharge again and on the lateral aspect of the heel a growth appeared. The part was very tender but not painful, The discharge became profuse and foul smelling. A biopsy of the growth revealed a squamous papilloma with low grade malignant change. There was much scarring around ankle. The extensors of toes and tendo-achillis had been severed and little movement was possible at the ankle joint.
Radiographs showed chronic osteomyelitis of the left calcaneum with considerable sclerosis and old infective arthritis of the ankle joint with new bone formation along the lower aspect of the fibula. The leg was amputated and an enlarged lymph gland in the groin excised. The gland showed no evidence of invasion, only chronic inflammatory reaction.
