History isn't known, but can see that the pelvis is uniformly dilated and the parenchym reduced to a thin sheath.
Microscopic examination would show hylinisation, scarring and crowding together of glomeruli. The interstitium would contain much fibrous tissue and cellular infiltrates, the quantity and nature of which would depend on the presence or absence of infection within the hydronephrotic sac. in this case the pelvic lining shows only a little infection and no exudate apart from some blood clot.
Hydronephrosis is most commonly caused by an obstruction to urine flow, which - as in spina bifida or paraplegia from other causes - may be a functional one. In a minority of cases noonstruction can be found and it is assimed that some defect in the ureter must prevent the transmission of urine downwards. Attempts to demonstrate the nature of this defect have been unsuccessful
