Clinical history:
A man of 63, an arc-welder, had complained of lassitude and anorexia for 3 months; there was some weight loss and anaemia, and the E.S.R. was persistently raised. Malignancy was suspected and the chest, stomach and colon were investigated radiologically but showed no abnormality. He died suddenly the day before he was due to be admitted to hospital for further investigation.
At necropsy the only significant abnormalities were found in the adrenals. The right adrenal weighed 6.0 g and the left 5.4 g, i.e. they were a little above average size. Both had lost their normal appearance and were greyer and firmer than normal.
On microscopic examination both showed granulomatous inflammation with some fibrosis and a few atypical giant cells in the chronic inflammatory infiltrate. There was no caseation and no tubercle bacilli or other pathogens could be demonstrated. No tuberculosis was detected elsewhere.
This is an unusual case of granulomatous adrenalitis of unknown pathogenesis. It does not conform with the appearances of the more usual causes of Addison's disease - viz tuberculosis, or the adreno-cortical atrophy which sometimes is associated with antibodies to cortical tissue.
