From a nullipara aged 45 years. She had noticed four months ago, a swelling of the thyroid gland which had rapidly increased. There were no toxic symptoms, but a feeling of constriction of the throat on exertion and slight dysphonia, Clinically the left lobe was hard. The right, enlarged, was of normal consistency. The thyreoid moved freely on deglutition and there were no palpable lymph nodes. The pulse varied from 70 to 90.
The lateral half of the surface of the lobe shows two nodules; one marginally, which is solid, and the other centrally, which is smooth, hemispherical, and of a purple colour. On section the parenchyma of the isthmus appears of normal thyreoid tissue. In the lobe no normal thyroid tissue is observable. The lobe is enlarged, dense, and except for a central necrotic area, appears homogeneous. On microscopic examination the appearances somewhat resemble a reticulum-cell lymphosarcoma, or even a ligneous thyroiditis. There is acinar (vesicle) formation of an abortive kind of tumour cells. These vesicles contain in some cases a little colloid, and might be original vesicles overwhelmed by tumour. In places there is some papillary arrangement, but on the whole the tumour is a diffuse small-cell thyreoid carcinoma. Lymph nodes examined showed simple hyperplasia with prominent germ centres and an endothelial reaction.
