Male age 74 years
Pain in left shoulder associated with climbing stairs for 4 months caused by ischaemia. Patient blind. BP 180/95; ECG normal; WR negative. X-ray showed slight pulsation of the smooth round lesion above the left hilum. It was difficult to say whether this was transmitted pulsation or true pulsation, but the former was the more likely. Both hemidiaphragms move 5 cms on deep respiration. There was no paradoxical movement on sniffing. Barium swallow was normal. The findings are slightly more in favour of aneurysm of the sinus of Valsalva or a mediastinal cyst rather than a bronchogenic carcinoma.
