From a male aged 42 years who had had defective vision from childhood and had been an inmate of an asylum for the blind since the age of 14 years. He had from childhood a patch of opacity in the right lens with old disseminated choroiditis and in the left eye an extensive detachment of the retina rather shallow and at parts suggesting retinitis proliferans. Fifteen years later the right eye had undergone very little alteration but the left fundus was no longer visible owing to secondary degeneration of the lens and it had no perception of light. Latterly the left eye became painful especially on exposure to wind. There was general injection with a rather contracted ovoid pupil. Several leashes of dilated vessels appeared on the surface of the iris a pinhead of yellow exudate near the pupil edge in the lower quadrant and in the same quadrant a wreath of exudate on the iris surface near the angle of the anterior chamber.
The detachment of the retina is very obvious, the lens opaque and there is evidence of inflammation about the iris. Microscopically sections show congestion and. some inflammatory reaction of the chorioid and retina. There is much congestion of the Chorioid with fibrosis and calcification anteriorly at the base of the ciliary body. A certain amount of congestion and inflammatory reaction of the conjunctiva is present at the circumference of the cornea but the cornea is not affected.
