From a male aged 3¼ years who was admitted to hospital with sore throat, vomiting, shivering and noisy respiration, of two days duration. Some eighteen months previously be had had difficulty in breathing and similar attacks occurred when be cried. There was no evidence of diphtheria, and repeated bacteriological examination proved negative. The dyspnoea was relieved by intubation and the tube had to be retained. Five days later the intubation tube was coughed out, and as there was difficulty in replacing it, tracheotomy was performed. Repeated attempts to re-insert the intubation tube failed, and after a lapse of five months an examination under general anaesthesia showed a normal rima glottidis with close approximation of the vocal folds and apparently an impermeable obstruction distally. Some three months later, on account of repeated cyanosis due to difficulties with the tracheotomy tube, the tracheal wound was enlarged and the trachea seen to be normal as far as the bifurcation but proximally completely blocked. Proximal to, and including, the vocal folds the larynx is normal. Thence to the tracheotomy wound there appears to be a solution of continuity [ between the larynx and trachea, the walls and lumen being replaced by a mass of cicatricial fibrous tissue continuous with a mass of subglottic fibrous tissue. Microscopic examination of the destructive area showed fibrous tissue with much lymphocytic infiltration and undergoing myxomatous change. The blood vessels showed the proliferative endarteritis of a chronic inflammatory process.
