From a male aged 23 years who had had discharge from his left ear some six years previously, but none recently. Pain in the neck was succeeded in four days by an abscess over the mastoid process. This was opened, but five days later symptoms of meningitis set in. The mastoid air-sinuses were opened but he did not regain consciousness and died the same evening. Post-mortem examination showed basal meningitis and an abscess of the temporal lobe of the brain.
While opening the mastoid air-sinuses, perforation occurred through the tegmen tympani and the overlying dura mater, causing septic leptomeningitis which proved fatal.
The brain substance at the site of the small left temporal abscess is softened and necrotic. Portions of the temporal lobe have been removed to show its extent.
