From a male aged 46 years who three weeks before his death had influenza followed by pain in the right ear and a slight discharge from the acoustic meatus. Examination after syringing showed a bulging membrana tympani through which pus escaped with each arterial pulsation. On the following day operation for acute septic mastoiditis showed pus in the mastoid air-sinuses and tympanic antrum (streptococcus haemolyticus on culture). Through bone which seemed normal, part of the dura mater of the middle cranial fossa was exposed and appeared healthy. On the following evening the temperature rose to 39.5º C, and two days later basal meningitis was evident. The radical mastoid operation was then carried out, with double vestibulotomy and the internal acoustic meatus was opened through the medial wall of the vestibule. A free flat of cerebrospinal fluid was obtained and 50 c.cm. antistreptococcic serum administered. Two days later with coma and a temperature reaching 40.5º C. ,he died. Post-mortem examination showed generalized purulent meningitis.
The brain is congested especially on the right side an. there are many areas of seropurulent exudation more an the right side than on the left. The posterior half of the inferior surface of the right temporal lobe is particularly affected.
