From a male aged 55, a trawl fisherman who, for five years had increasing hoarseness. There had been no pain and no trouble with the ear or throat. Occasionally he had a choking sensation when eating nuts or apples. When first seen two years ago both cords showed superficial ulceration and elsewhere the epithelium was thickened. Movements of the cord were free and there was nothing abnormal in the neck, nose, ears, pharynx or chest, and the Wassermann test was negative. The condition was considered to be leukoplakia and this diagnosis was strengthened by the report on a biopsy. The epithelium was hyperplastic with excessive and disordered keratinization. The cells were regular in size and there was no evidence of invasion of the deeper tissues. In places the deeper layers of the epithelium were broken up by infiltration with lymphocytes and plasma cells. The patient was examined every few months and on the last occasion the right cord was oedematous and fixed. A biopsy confirmed the diagnosis of squamous epithelioma. Total laryngectomy was done and an enlarged gland lying on-the crico-thyroid membrane, and several smaller lymph glands were removed. A small vessel and an aberrant branch of the superior laryngeal nerve passed through a foramen of the thyroid cartilage on each side. The enlarged gland on the crico-thyroid membrane was found to be invaded with tumour, but other three lymph glands showed only chronic inflammation.
The larynx has been laid open by a posterior vertical incision. An ulcerative lesion involves the whole of the right vocal cord and extends down about 1.8 cm. into the subglottic region and slightly across the mid-line in front. The surface of the ulcer is irregular, the margins raised, particularly in the region of the vocal cords. An area of hyperkeratosis extend-s from the ulcer along the left vocal cords. Elsewhere the laryngeal mucosa is congested. Near the posterior border of each ala there is an aperture which transmits a small vessel and an abnormal branch of the superior laryngeal nerve.
