From a male aged 62, a plasterer, who for six weeks complained of hoarseness and of pain on the left side of neck radiating to the ear. There was also occasional dysphagia. On laryngoscopy the hypopharynx and upper part of the oesophagus were normal. The whole of the left vocal cord was replaced by an ulcerative growth which did not extend on to the false cord. A biopsy showed a squamous carcinoma with some keratinisation. The larynx was resected.
The larynx has been laid open posteriorly. A typical ulcerative squamous carcinoma involves the whole length of the left vocal cord and posteriorly has extended a short distance on to the arytenoid and downwards for about 9 mm. into the subglottic region. The ulcer has an irregular base with raised margin.
