From a male aged 59 who, eight years previously, sought advice because of recurrent laryngitis of four or five months duration. The vocal cords showed ulceration and the Wassermann reaction was strongly positive. He was given treatment for syphilis and the laryngeal condition improved. Three years ago he again reported because of persistent hoarseness and examination revealed marked leucoplakia of both cords. The W.R. was still positive. Thereafter he was examined periodically. There was no respiratory difficulty and he remained in full employment. The laryngeal condition appeared to be remaining stationary. Areas of sloughing, however, developed on both cords. A biopsy was done and this was reported as leucoplakia with malignant change. Laryngectomy was performed from which the patient made a good recovery.
The larynx has been laid open by a posterior vertical incision, The whole circumference of the rima glottidis is affected with leucoplakia. The vocal cords are obscured and the lesion extends into the subglottic region so as to form a circular zone measuring 1 cm, vertically. The keratinized. layer is heaped up in the form of crowded white nodules. A vertical ulcer anteriorly is the site of the biopsy. There is no definite visual evidence of deep invasion on the cut surface of the larynx. A photomicrograph of the biopsy shows extension of the squamous epithelium into the corium arid much lymphocytic infiltration. Mitotic figures in the epithelial tissue are numerous.
