TitleSarcoma of the larynx
DescriptionSarcoma of the Larynx.
Incidence. One half to one per cent of all malignant tumours of the larynx.
Histological types. Spindle celled tumour, osteosarcoma, fibrosarcoma, myosarcoma, leiomyosarcoma.
Clinical features. At first smooth, or lobulated, in early stages, in later, ulcerated and rapidly growing.
Spread. Metastases are late. Glands involved in 15% (Ewing)
Prognosis. Rather more favourable than in carcinoma.
Treatment. In localised tumours - surgical removal is recommended, in others radiation.
Present Case. Male aet. 46. Occupation: lorry driver.
First complaint: ‘laryngitis” for six weeks.
Examination: proliferating tumour found involving the whole length of the right vocal cord.
Movement of the cord was good. There was no subglottic spread.
Histological Report. Poorly differentiated growth suggestive of carcinoma.
Treatment Ny linear accelerator.
Progress. Three months after treatment (12.9.57.) the right vocal cord was normal and movement was
normal.
Two months later (14.11.57.) a smooth reddish swelling was seen on the right vocal cord.
After another two months (2.1.58.) there was a large mass on the right vocal cord obstructing the airway. There were no palpable glands.
Histological report. Spindle celled sarcoma.
Operation. Laryngectomy (4.2.58.) with block dissection of glands on the right side.
Microscopic examination of the glands showed a suspicious deposit in one gland.
Recovery. patient made a good recovery and was
discharged from hospital (9.4.58.)
