Left hemisphere of centrum and cerebellum showing a craniopharyngioma.The outer surface of the brain shows marked congestion, numerous haemorrhage of small size and flattening of the gyri. Viewed from the medial aspect the tumour presents lying anterior to the pons and cerebral peduncle. Superiorly it lies immediately below the corpus callosum and anteriorly in relation to gyrus subcallosum, Inferiorly the tumour is visible at the base of the brain in close relation to the tip of the temporal lobe and the optic nerve. The tumour is well-defined posteriorly but appears to extend posteriorly beneath the splenium. Anteriorly it has invaded the rostrum and genu of the corpus callosum and forms an irregular lining of the ventricle in this area.The tumour is irregular in shape but roughly triangular, measures approximately 5 cms. by 4 cms. in the vertical and horizontal diameters. The cut surface shows areas of solid homogeneous tumour tissue with superiorly a large haemorrhage, whilst the most inferior part of this tumour is cystic with intracystic irregular papuliferous formation.
A typical craniopharyngioma growing in a cyst bounded by a thin fibrous capsule.Its structure resembles that of an intra-alveolar adamantinoma, with branching anastomosing epithelial columns of poorly differentiated squamous cells, limited by palisaded columnar cells. There is extensive hyaline degeneration with much calcific deposit, almost to bone formation. There are areas of cholesterol crystal clefts in the supportion tissue. The adjoining cerebral tissue shows early invasion.
