John Cleland was born in Perth where his father was a medical practitioner. He was educated in Edinburgh at the High School and the University. He first came under the influence of John Goodsir when he was a student and later, after graduating M.D. in 1856, he was appointed junior demonstrator in Anatomy under Goodsir with William Turner as his senior.
In 1863 he was appointed Professor of Anatomy and Physiology at Queen's College, Galway, a post he held until 1877 when he succeeded Allen Thomson as Professor of Anatomy at Glasgow University. While he was at Galway he was Medical Officer to the Workhouse Infirmary as well as holding his University post. By 1865 some of the Poor Law Guardians were complaining about the increased cost of keeping patients in the Infirmary and the amount of money spent on surgical instruments and new fittings but the majority praised him for the marked improvement in the Institution under his guidance.
While at Galway he engaged actively in research and published many monographs on a wide range of subjects which included the ligaments of the phalanges and the descent of the caecum.
He was the author of various books on anatomy such as the ‘Directory of Dissection’ (1876) and Cleland and Mackay's ‘Human Anatomy’ (1896) which is believed to be the first textbook of Anatomy to use photographic illustrations. The Preface states: "Photography has been largely made use of in the section on the Skeleton. It is hoped that by this means accuracy has been gained, while distinctness has been secured by the aid of touching, often of an elaborate kind, which had to be taken in hand by Dr Cleland,"
