At the base of the brain, especially in the interpenduncular space, the pia-arachnoid is opaque and matted and there is a grey-ish yellow exudate which spreads on to the orbital surface alongside the blood vessels. Minute grey tubercles are visible among the exudate but are more readily distinguished where it is less abundant, e.g. on the temporal poles, orbital surfaces and around the outer part of the lateral fissures. There is a certain amount of general meningeal congestion. The cerebral convolutions are flattened. The lateral ventricles are moderately dilated.
Post-mortem note:
Caseous tuberculous lymph nodes in mesentery. Cervical and thoracic lymph nodes healthy. Hardly any miliary tuberculosis.
